Making Appointments

  • New Patients: New patients please schedule a 90-minute initial visit. New patient forms need to be completed at least one week prior to appointment to allow time for review. A credit card is required to book and subject to cancellation policy.  

    Payment Policy

    A credit card is required to make appointments and will be charged unless another form of payment is agreed upon prior to appointment. Please email to arrange alternative form of payment.

    Cancellation Policy
    To cancel or reschedule an appointment: patients must give four calendar days notice to either cancel or reschedule an appointment without charge.  If less than four days notice is provided, clients will be charged 50% of the full appointment fee. If cancellation is made same day, the full appointment will be charged. We require that all patients have a current credit card on file.

Insurance Policy

  • We do not accept any health insurance plans. After the visit, (non-Medicare) patients with PPO insurance will be given a superbill with appropriate CPT/ICD10 diagnostic codes for them to submit to their insurance plan to seek out-of-network reimbursement (directly to patient).

  • Patients covered by Medicare, Medicare supplemental, or Medi-Gap insurance policies will be given a simple receipt (without CPT/ICD10 codes) and will not be able to submit any claims to their insurance policies for our services.

  • After telephone/ Zoom appointments patients will automatically be emailed a simple receipt (without CPT/ICD10 codes) when charged since these visits are not usually covered by insurance.
